Step three. What documents do I need to submit?
1. An application, including your name, surname, personal code, place and date of birth, place of residence, place of work, marital and financial situation), family members, the name and surname of the spouse’s child to be adopted, the date of birth, the state of health of the child, details of the child’s parent who is not fulfilling the obligation to raise the child and who has therefore made the child available for adoption, and the grounds for adoption.
2. A health certificate (046/a) in the form prescribed by the Minister of Health, issued by a medical advisory board.
3. A copy of the birth certificate of the child to be adopted.
4. A copy of the court decision on the indefinite limitation of the child’s biological father’s (mother’s) authority or the court order on consent to the adoption of the child; a copy of the father’s (mother’s) death certificate.
5. Written consent (free form) from the spouse regarding the adoption of his/her child.
